A claim is filed with the carrier that wrote the policy, not with the agency. The agency's job is to make sure you are filing it in the right place, then stay involved while the carrier handles it. Call before you search for a claims number online.

What to include in a certificate request.
A certificate of insurance goes out faster when the request already has everything the general contractor or property owner is going to ask for. Send these four things with the request:
The job name
So the certificate can be matched to the right project.
The certificate holder's exact legal name and address
A close guess is not good enough. Property managers and general contractors reject certificates over a mismatched name or address.
The limits required
If the job has a contract that states minimum coverage limits, send that page along with the request.
Whether additional insured or waiver of subrogation is being demanded
These are separate endorsements from the certificate itself, and leaving one off a request is the most common reason a certificate has to be reissued.
